The Monitor sent correspondence to all HCPH retirees on May 6, 2010. In accordance with the Retiree Audit Survey Process that was approved by the Court on April 15, 2010, all retirees will receive a Personal Information Statement by mail. The Personal Information Statement will be used to confirm the accuracy of your current pension and benefit profile with the company. You will also receive an instruction sheet along with your Personal Information Statement, which will ask you to review your profile and to make any corrections that may be necessary. You are required to complete and return this form to the Monitor by June 18, 2010. If you do not return the form by June 18, 2010, you are deemed to accept the information contained in the Personal Information Sheet.
